For an application to be lodged, the following documents must be completed:

• Application Form
• A Parish Priest Reference form to be completed by your Parish Priest (if applicable)
• Data Collection Form
• School Entry Health Assessment Form
• Medical History Report Form
• Collection of Information Disclaimer
• Internet, Publicity & CEWA account
• Kindergarten Student Health History Form (Kindergarten students only)

The following documents must also be submitted:

• Baptism Record (if applicable any denomination)
• Child’s Birth Certificate
• Immunisation History Statement from Medicare

  • If the student and/or both parents were born overseas, a copy of the student and parent’s Visa, Passports or Citizenship, please

A $55 non-refundable application fee is due per student on acceptance of a position.
